2) Legal Name (as shown on your ID)

3) Your Email

4) Your Phone

Conditions:

I cover the cost of the hotel room.

You will be responsible for

the room tax,

resort fees (if any) and your

own transportation,

food and activities.

The total room tax is payable at the time of

activating the hotel voucher

(the per night price is shown next to the hotel options).

Once I have the voucher sent to you,

you have 7 days to activate it and

18 months to use it.
